I don't know whether Semenya is infertile. It wouldn't be unusual for someone with a DSD.

Probably because it's very difficult to figure out how Semenya's system will respond to particular medication: you have a highly trained athlete plus an unusual metabolism.

Transgender people undergoing transition are completely standard metabolisms, for the most part, and there's decades of medical experience about how to administer cross-sex hormones. So it's not in the least surprising when you consider the relative circumstances.
